This Consumer Health Data Privacy Notice (“Notice”) supplements the Fox & Cub Family Wellness Privacy Policy and is effective as of March 1, 2025. Our Privacy Policy explains the personal information we collect and how we use it. This Notice is provided in compliance with the My Health My Data Act (“MHMDA”) in Washington and Nevada’s Consumer Health Data Privacy Law (“NV CHDP”) and applies only to personal information that may be considered “Consumer Health Data” under these laws.

This Notice does not apply to medical records, diagnoses, or any other form of Protected Health Information (PHI) covered under HIPAA or other healthcare regulations.
