Birth and Postpartum Doula Matchmaking Agreement

This agreement outlines the terms under which Fox & Cub Family Wellness, Inc. (“Fox & Cub”) will provide doula referral and matchmaking services for the undersigned client(s). Fox & Cub is not a direct provider of doula care. All doula services are delivered independently by the doula you choose to contract with.

Nature of Fox & Cub’s Services

Fox & Cub serves solely as a referral and matchmaking agency. We introduce families to qualified doulas and assist in the initial connection process. Once you choose a doula, all ongoing services, terms of care, fees, and scheduling are handled directly between you and your doula.

Limitation of Responsibility

Fox & Cub does not supervise, employ, or control doulas. We are not responsible for the actions, omissions, style of support, or outcomes of care provided by any doula. Doulas are independent professionals who contract directly with you.

Acknowledgment of Client Responsibility

By signing this agreement, you acknowledge that:

Any payments, retainers, or contracts beyond Fox & Cub’s referral fee are between you and your doula.

Fox & Cub’s role is limited to referral and matchmaking only.

Your doula is an independent provider responsible for all aspects of your care.

Continuation of Care

If you wish to extend or renew services with your doula, please let us know in case your doula is not available for any new hours you will need support, we are happy to find you a replacement doula.
