Contract Applicable to All Services

This Service Agreement ("Agreement") is made between Fox & Cub Family Wellness ("Company") and the undersigned individual ("Client"). By signing this Agreement, the Client acknowledges and agrees to the terms and conditions below related to a service package purchased through our website. Services may be provided directly by Fox & Cub Family Wellness or by one of our approved independent contractors, depending on availability and client needs.

I. General Service Terms (Applicable to All Packages)

Scope of Services

  • Services are educational and supportive in nature and do not replace medical or mental health care.
  • Support may include lifestyle coaching, postpartum recovery guidance, feeding support, and sleep strategies.
  • Scope of Support & Coaching Disclaimer:
    Some wellness coaching services may be delivered by individuals who hold advanced professional licenses (e.g., LCSW, LMFT, RN). However, in the context of your relationship with Fox & Cub Family Wellness, these individuals are not providing clinical therapy, medical diagnosis, or treatment.

All services offered through our platform are delivered under a health & wellness coaching model focused on lifestyle, emotional resilience, parenting support, and wellness education. If at any point clinical care is needed or requested, a separate referral to a licensed provider outside of Fox & Cub Family Wellness will be made.

Provider Responsibilities

  • Respect your family, home, and parenting style.
  • Deliver non-medical support tailored to your selected package.
  • Offer resources and tools based on training and best practices.
  • In the event of emergency or illness, rescheduling will be arranged if possible.

Client Responsibilities

  • Communicate needs before and during sessions.
  • Actively participate in sessions.
  • Provide 24 hours’ notice for reschedules (no refunds for no-shows). A one-time exception will be made for rescheduling an appointment within the 24-hour window in the case of emergency/unexpected situations.
  • Acknowledge services are not medical advice or clinical care.

Mental Health Disclaimer & Emergency Support

Fox & Cub Family Wellness and its independent contractors are not acting as a licensed psychotherapist or mental health clinicians, even if they have an outside licensed psychotherapy practice. The services provided—including Health & Wellness Coaching, Lactation Counseling, Pediatric Sleep Consulting, and Postpartum Doula Support—are non-clinical in nature and are not a substitute for psychotherapy, counseling, or licensed mental health treatment.

You are not alone, and support is available.

II. Independent Contractors

  • Services may be performed by approved independent contractors aligned with Fox & Cub Family Wellness values.
  • Clients will be informed of their assigned contractor.
  • Since our consultants are not our employees, they may have their own contracts that they send you with their own agreements.

If a Fox & Cub client refers someone to a contractor, the referral must go through Fox & Cub.

III. Cancellations & Rescheduling

  • Cancellations are not permitted but may be rescheduled with 24 hour notice.
  • Clients may reschedule with 24+ hours' notice.
  • Missed appointments without notice will not be refunded or rescheduled, and may include an added fee per the consultants own contract.

We are committed to providing the highest quality support to every client. However, we reserve the right to respectfully decline or discontinue services at our discretion if we determine that our offerings are not the right fit for your needs. In such cases, any payments made will be fully refunded.

IV. Confidentiality

  • All client information is kept confidential unless disclosure is legally required or consent is given.

Confidentiality & Electronic Communication

All information shared by the Client during the course of services provided by Fox & Cub Family Wellness—whether through coaching, consultations, doula support, or any other service—will be treated as confidential to the extent permitted by law. Personal or sensitive information will only be disclosed with the Client’s written consent, or as required by law, including but not limited to situations involving potential harm to self or others (such as suicidal or homicidal ideation), child abuse, or a lawful court order, in accordance with California state laws.

Please note: Some of our consultants may have additional confidentiality requirements or practice-specific policies outlined in their own service agreements. By receiving services from these professionals, you agree to review and accept the terms presented in their individual contracts, which will be provided to you prior to the start of services.

The Client acknowledges that services may be provided in person or through electronic communication (such as phone, email, video conferencing, or messaging apps), and that these communications may involve the sharing of personal health or lifestyle information. The Client is responsible for ensuring their own privacy during such sessions and communications, and for participating in an environment where they feel secure and comfortable.

Fox & Cub Family Wellness does not record any sessions or communications, whether virtual or in-person. Clients are also expected to refrain from recording sessions unless explicitly authorized by Fox & Cub Family Wellness in writing.

Risks

While the services provided by Fox & Cub Family Wellness are designed to be supportive, educational, and collaborative, the Client acknowledges that engaging in emotional, personal, or wellness-related discussions may sometimes bring up challenging thoughts, emotions, or memories.

The Client agrees to participate at their own pace and level of comfort and is encouraged to pause or discontinue a session at any time if needed.

If at any point it becomes clear that the Client may benefit from specialized mental health support, therapy, or clinical intervention beyond the scope of the services provided, the Client agrees to seek care from a licensed mental health professional. Fox & Cub Family Wellness reserves the right to refer the Client to a licensed professional or appropriate emergency services if it is determined that continuing services may be outside of the Company’s scope or if there is a perceived risk of harm to self or others.

V. Liability & Medical Disclaimer

  • Fox & Cub Family Wellness does not diagnose, treat, or offer clinical care.
  • Clients assume responsibility for all decisions and outcomes related to services received.

VI. Additional Service-Specific Terms

A. Health & Wellness Coaching

Clients engaging in Health & Wellness Coaching will be matched with a coach based on their individual needs and wellness goals. Each independent contractor may specialize in different areas of health and wellness, including but not limited to fitness planning, nutrition, emotional well-being, or lifestyle balance.

Scope of Services may include the following:

  • Personalized coaching sessions to assess lifestyle, challenges, and health goals
  • Guidance on stress management, nutrition, movement, and self-care practices
  • Support in creating sustainable habits for long-term wellness
  • Customized action plans and strategies for achieving individual health objectives

Health & Wellness Coaching is intended to support general well-being through education, motivation, and accountability. It is not a substitute for clinical therapy, medical nutrition therapy, or mental health treatment. Clients are responsible for seeking appropriate licensed medical or mental health services when needed.

1:1 Coaching Session Policy

Health & Wellness Coaching sessions at Fox & Cub Family Wellness are designed to be private, individualized, and deeply personal. To maintain the integrity of our work together and to foster a safe and focused environment, all coaching sessions are one-on-one and intended for the client only.

No additional individuals—such as partners, friends, or family members—may be present during coaching sessions, whether held in person or virtually. This ensures that each session remains centered on your personal goals, challenges, and growth without distraction or external influence.

If you have questions or concerns about this policy, we’re happy to discuss how to best support your experience while maintaining appropriate boundaries.

Bringing Your Baby to Sessions

At Fox & Cub Family Wellness, your baby is always welcome—especially during the early postpartum period and while breastfeeding. We fully understand that arranging childcare isn’t always possible, and we would never want that to become a barrier to accessing support. Whether your session is held online or in person, you are welcome to have your baby with you.

Package Options:

Foundations of Wellness – $199

  • One 60-minute Zoom session
  • Lifestyle and health goal assessment
  • Personalized wellness plan for the next 4 weeks
  • This package includes a one-time consultation and a customized wellness plan designed to guide the client through the following 4 weeks. The wellness plan may focus on one or more of the following areas: physical activity, nutrition, emotional well-being, or overall lifestyle improvement. No additional follow-ups or check-ins are included with this package unless separately arranged.

Transformational Wellness – $796

  • Four 60-minute weekly virtual coaching sessions conducted via Zoom
  • In-depth assessment and personalized goal-setting
  • Habit-building and lifestyle improvement strategies
  • Nutrition, energy, stress, and wellness support
  • Ongoing messaging support between sessions for clarifying guidance
  • Chat support via your coach’s selected coaching app Monday–Friday, 9 AM–5 PM PST (messages responded to within 3 hours; after 2 PM PST, replies may come the next business day)
  • The four coaching sessions will take place once weekly via Zoom. Goal-setting and coaching plans are designed to support the client throughout the four weeks following the initial Zoom session.

3-Month Wellness Coaching Package (Six 1-Hour Virtual Sessions) - $1194
 
Six 1-hour private virtual coaching sessions (biweekly over 3 months)

  • Matched with a coach who specializes in your specific needs and goals
  • Personalized action plan based on your health priorities and lifestyle
  • Support may include nutrition guidance, fitness planning, stress management, postpartum recovery, hormone balance, or lifestyle routines
  • Ongoing messaging support between sessions for clarifying guidance
  • Tailored resources, tools, and recommendations after each session

Between-Session Communication Policy – Transformational and 3-Month Wellness Package

Clients enrolled in the above two packages will have access to messaging support between scheduled sessions via email or the communications platform of the consultants choosing. This communication is intended to support your ongoing progress by answering brief questions or clarifying guidance related to coaching goals that have already been discussed.

Please note:

  • Between-session communication is not intended for introducing new goals or topics.
  • It is not a channel for urgent or emergency situations.
  • Any new concerns, challenges, or topics requiring deeper exploration will be addressed during your next scheduled coaching session.

This policy ensures that your coaching experience remains focused, intentional, and within the appropriate scope of support. If you ever feel unsure about whether your question fits within this boundary, feel free to bring it to your next session or ask for clarification during scheduled communication hours.

B. Lactation Counseling

Initial Consultation (90 mins) – $199

Follow-Up (60 mins) – $149

Client Agreement:

  • This agreement applies only to the selected package.
  • Additional sessions must be purchased separately.
  • No refunds after purchase.
  • Reschedules require 24+ hours' notice.

Client Responsibilities:

  • ✅ Follow the plan as best as possible
  • ✅ Understand services are not medical
  • ✅ Seek medical help if issues arise

Liability & Medical Disclaimer:

  • Fox & Cub is not a medical provider.
  • The consultant is not a doctor and does not diagnose or treat conditions.
  • Information provided is educational and supportive only.
  • The consultant is not responsible for any outcomes related to sleep, feeding, weight gain, or milk supply.
  • If medical concerns arise, the Client must consult a pediatrician, OB/GYN, or another healthcare provider.
  • The Client assumes all responsibility for decisions made based on the advice received during the consultation.

Confidentiality:

  • All client info is kept private unless written consent is given or required by law.

C. Pediatric Sleep Consulting

All packages include:

  • ✔ 30-minute phone consultation
  • ✔ Custom sleep plan
  • ✔ Daily schedule and nap video
  • ✔ Chat support via Rested (when included)

Response time: All chat/emails answered within 3 business hours Mon–Fri, 9 AM–5 PM PST. (except for Basic Sleep Package)

Comprehensive – $319

Gentle Sleep Approach & Crying Disclaimer

All sleep learning methods used by Fox & Cub Family Wellness are rooted in gentle, responsive approaches that prioritize your child’s emotional well-being and your family’s values. While some crying is a natural and expected part of any sleep transition, we do not support “cry-it-out” or neglectful methods.

Our goal is to help your child learn independent sleep skills while ensuring they continue to feel safe, comforted, and reassured throughout the process. You’ll receive guidance that aligns with your parenting style and supports a loving, secure attachment.

D. Postpartum Doula Support

Please note: We are currently only accepting families with singleton babies. We are not offering support for twins or multiples at this time.


Daytime Support – Single Baby

4-Hour Session Package (Can be virtual or in-person in Sonoma County, CA only)
Rate: $200 per session
Includes:

  • Newborn care
  • Feeding and lactation support
  • Light household tasks
  • Emotional support
  • Sibling guidance

Daytime Shift Scheduling:
Daytime support is offered in 4-hour sessions:

  • Morning: 7:00 AM – 11:00 AM or 8:00 AM – 12:00 PM
  • Afternoon: 1:00 PM – 5:00 PM or 2:00 PM – 6:00 PM

Overnight Support – Single Baby (In person in Sonoma County, CA only)

8-Hour Session Package
Rate: $480 per session
Includes:

  • Full overnight newborn care
  • Feeding support (breast/chest/bottle)
  • Parental rest support

Overnight Shift Scheduling:
Overnight sessions are scheduled for a minimum of 8 hours:

  • 9:00 PM – 5:00 AM or
  • 10:00 PM – 6:00 AM

Arrival Procedure:
When your doula arrives for an overnight shift, they will knock first. If there is no response, they will attempt to text and call you. If there is still no response within 10 minutes, they will leave, and the session will be forfeited with no refund or rescheduling allowed.

Rest Policy for Overnight Shifts:
Overnight doulas are permitted to sleep when the baby is sleeping, provided all baby care needs have been met. A flat, safe surface must be made available for the doula to rest—such as a couch, daybed, or inflatable twin air mattress, ideally in or near the baby’s room.


Welcome Home Visit (Virtual or in-person in Sonoma County, CA)

Duration: 2 hours
Rate: $250
A one-time session to ease the transition home from birth.
Includes:

  • Newborn care support
  • Feeding guidance
  • Postpartum recovery support
  • Emotional check-ins and reassurance

In-Home Baby Prep Visit (Virtual or in-person in Sonoma County, CA only)

Duration: 2 hours
Rate: $250
Scheduled around 35–36 weeks of pregnancy to help families prepare for baby’s arrival.
Includes:

  • Nursery setup guidance
  • Basic baby care education
  • Home safety tips
  • Preparing partners/siblings

Scheduling & Support Terms

  • After purchase, clients will be contacted within 24–48 hours to begin scheduling.
  • All support hours must be used within 3 months from the date of the first scheduled session.
  • Sessions are scheduled based on doula availability and client needs.
  • Services are available throughout Sonoma County, CA, including Santa Rosa, Petaluma, Sebastopol, Healdsburg, and surrounding areas.

Illness & Missed Appointments Policy

  • Clients must notify their doula immediately if anyone in the home begins to feel unwell.
  • With at least 24 hours’ notice, sessions may be rescheduled.
  • In cases of sudden illness or emergency, a one-time reschedule will be allowed within the 3-month support period.
  • If no one answers the door within 10 minutes of the doula’s arrival, the shift will be forfeited without refund or rescheduling.

If a household member is sick:
The doula is not required to return until everyone in the household is feeling fully recovered. Postponed sessions may be rescheduled within the original 3-month window.


COVID-19 Policy

  • If anyone in the household tests positive for COVID-19, the doula will not attend scheduled shifts.
  • Services will only resume once all household members (including frequent visitors) have tested negative for COVID-19.
  • Missed sessions due to COVID may be rescheduled within the original 3-month support window.

HSA Superbill Policy

If you plan to use your Health Savings Account (HSA) for reimbursement and did not use your HSA card at checkout, you must notify us within 7 days of purchase to receive a superbill.
Fox & Cub Family Wellness is not responsible for denied reimbursement. Clients must confirm postpartum doula care is eligible under their HSA plan.


Duties Not Performed by Doulas

Postpartum doulas at Fox & Cub Family Wellness provide non-medical support only. They do not:

  • Perform major housecleaning (e.g., mopping, scrubbing bathrooms, yard work, window washing)
  • Do major cooking or meal preparation for the entire household
  • Provide transportation to clients or family members
  • Conduct clinical tasks, medical assessments, or give diagnoses
  • Speak on behalf of the client in any healthcare or personal matter
  • Make parenting or healthcare decisions for the client

Doulas may support clients in finding resources and making informed decisions but do not offer medical care or act as clinical providers.


Non-Solicitation Agreement

Clients may not hire Fox & Cub doulas independently or outside of the Fox & Cub platform. If a client chooses to bypass this agreement, they agree to pay a $15,000 retainer fee to Fox & Cub Family Wellness for violation of terms.


Consumer Health Data Privacy Agreement

Fox & Cub Family Wellness values your privacy and is committed to protecting your personal and health-related information. In compliance with the California Consumer Health Data Privacy Act (and any applicable state or federal laws), we maintain a clear and transparent Consumer Health Data Privacy Policy, which outlines how your data is collected, used, stored, and shared.

By signing this Agreement, you acknowledge and agree to the terms outlined in our Consumer Health Data Privacy Policy, available on our website at foxandcubfamilywellness.com/privacy. This policy applies to all services provided by Fox & Cub Family Wellness and its independent contractors.

If you have questions about how your health-related data is handled, we encourage you to review the policy before proceeding.


Governing Law & Jurisdiction
This Agreement shall be governed by and construed in accordance with the laws of the State of California. Any legal action or proceeding arising under or in connection with this Agreement shall be brought exclusively in the courts located in Santa Rosa, California.

VII. Acknowledgment

By signing this Agreement, you confirm your understanding and agreement with all terms listed above. Fox & Cub Family Wellness and its providers are not liable for any undesired or unexpected outcomes.
